The second link is to a letter which was issued by the Department
of Health and Human Services (Center for Medicare and Medicaid)
on May 6, 2005. This is a real letter and the phone numbers and
websites referenced have been verified to be legitimate.

Please notice that the NPI number is mandatory for dentists who
use Electronic Insurance Claims. If you do not submit claims to your
insurance companies electronically, you may still have to apply for
this number to replace any "identification numbers" you use for
your paper transactions.

For further information specific to your insurance companies,
please
contact your insurance provider.

NOTE: There is no fee and no handbook required to apply for this
number. If you receive anything in the mail urging you to purchase
materials for this number, please disregard them and call MSDA,
the ADA or the Department of Health Services to verify.
